We must receive certain documents from you before you start working with ATC Healthcare. You will jump-start the process if you can get these documents together while you are awaiting your first call.
Please see the following list for needed documents:
- Two Forms of US Citizenship or i9 identification
- Professional State Licensure information (ex: LPNs, RNs, PTs, etc.)
- CPR card and any other related certifications
- TB skin test (within the last 6 months) or Chest X-Ray (less than or equal to a year old)
- Health/Physical Statement (Doctor or Nurse Practitioner must provide and sign)
